A 4 part toolbox meeting form which allows the safety officer or Person In Charge to document discussions and important information during the toolbox meeting
The Tool box meeting form is a daily form which documents the project information such as project name, work location and work type. Date & time will be generated from the system to prevent cheating and the conductors information are also recorded. The form then allows documentation of topic discussed followed by the attendees name along with comments/remarks from the attendees. The form should be attached to a Permit To Work once it is completed.
