This is a 3 part lifting operation checklist for applicants to fill in important details regarding documentation and pre-lifting operation checks.

The lifting operation checklist ensures that the required documentations such as machinery cert validity, relevant permits & pre-requisite documents are available & verified before work can begin.

Pre-lifting operation checks such as daily machinery & maintenance history checks, lifting gear physical and colour coded checks, environmental & communication checks along with supporting tools availability are enforced with this checklist.

Once this checklist is completed, it should be routed for approval before attachment to a Permit To Work"
