This is a 4 part lifting checklist/form which allows the applicant to document essential information of the lifting plan for approvals.

The checklist is a detailed documentation of the lifting plan. The first portion of the plan includes generic information about the project, lifting location, company, classification of lifting operation and date and time information along with the endorseer and approver information. The second portion of the plan takes into account the details of the lifting machines, lifting gear and load such as weights, lengths and safety standards. Thirdly, the means of communications are also documented The last portion is an attachment of all essential documentations such as machinery & personnel certifications, plan views, lifting point diagram and crane diagrams. Once completed, the lifting plan should be routed for endorsement and approval before attachment to a Permit To Work.
